We are encouraging our athletes to invite at least 3 people to participate in our fundraising 5k. We are offering that for each reference the athlete will get $10 towards their sports fees in the Spring. As you know, our HS sports fees are $400 and MS sports fees $200, so every reference is a big help towards helping Mapleton athletes! Please make sure to find who invited you to run this race from our athlete roster in the drop down box. Click their name and make sure they get the credit for inviting you!

I the undersigned realize that participating in this event is a potentially hazardous activity. I should not enter and Participate unless I am medically able and properly trained. I agree to abide by any decisions of a race official relative to safely completing the race. I assume all risks associated in running this event including but not limited to falls contact with other participants the effects of the weather traffic and the conditions of the road all such risks being known and approved by me. Having read this waiver and knowing these facts and in consideration of you accepting my entry I for myself find anyone entitled to act in my behalf waive and release The Mapleton Cross Country Program The Polk Lions Club Mapleton Athletic Department all sponsors and any representatives associated with holding the race from all claims of liability of any kind arising out of my participation in this event. We reserve the right to reject entries. Race entries may be nonrefundable.
